This chapter describes Mothering from the Inside Out (MIO), a mentalization-based individual therapy for mothers with substance use disorders. Originally developed for mothers enrolled in outpatient substance-abuse treatment caring for children aged newborn to three, MIO is a short-term, supportive, individual psychodynamic therapy for substance-abusing mothers that emphasizes the development of the capacity for mentalizing. The chapter first provides an overview of MIO's conceptual basis, its efficacy, and mechanisms of change before turning to the mentalizing of transference and countertransference. It then considers the Parent Education program, in which the mothers meet individually with a parenting specialist to address their specific personal or parenting concerns. It also discusses MIO's other features, including developmental guidance and assessment, clinical supervision, referral and follow-up and concludes by summarizing the program's preliminary findings and outlining directions for future research.
